New Patient Registration

After booking a new patient appointment, you’ll receive a text message and/or email with a link to your registration forms. We recommend completing these forms as soon as possible as it will save you time on the day of your first visit.

Your First Visit

Please bring your dental insurance card if you’ll be using insurance. If you haven’t completed your new patient registration ahead of time, please arrive 15 minutes early and have your current medication information available.

Patient Portal

Our patient portal gives you secure online access to upcoming appointments, statements, and payments. If you don’t already have an account, we’ll email you an invitation seven days before your appointment with a link to set up your username and password.
